To change your mailing address or name on-line with the Bureau of EMS, go to: https://ww2.doh.state.fl.us/mqaservices/login.asp and follow the steps:

1. Choose profession: Paramedic or Emergency Medical Technician
2. If you have registered before:
   • Enter Account/User ID
   • Enter Password
3. If you have never registered on-line before:
   • Click link: Forgot Login Information
   • You will be prompted to forward a request via e-mail. Include your:
     1. Name
     2. Profession
     3. Certification number
     4. Last 4-digits of your social security number
     5. Date of Birth
     6. Current mailing address
     7. Reason (state to establish e-mail account)
